HP Quad Port NC382i lost 2 ports after reboot HP ProLiant 380 G7 devices still visible however

Hello all,

After a reboot I've lost 2 nics from available network adapaters on a quad port NC382i.  I was initially running ESXi4.1 U1, and have since updated to U3 and installed the lastest firmware/bios for the server (HP ProLiant DL380 G7).  When running an lspci I am able to see all nics on my quad, however they show up as BCM NetExtreme II BCM5709 on bus 3, where the other two ports are on bus 1.  Not sure what I'm missing here, the server was fine for over a year without issues.

According to the HCL (http://www.vmware.com/resources/compatibility/detail.php?deviceCategory=io&productid=4623&deviceCate...) it looks like the driver was included ("inbox") with ESXi 4.1 until Update 1 and then has been removed from the installation image ("async). You may need to manually install the bnx2 driver which can be downloaded from https://my.vmware.com/web/vmware/info/slug/datacenter_cloud_infrastructure/vmware_vsphere/4_1#driver...

I did actually use the latest drivers prior to trying a fresh install, and the odd thing is two of the ports on the adpater show up fine and I can see them in Network Adapaters as available vmnics.
